NW Sports & Performance Medicine is a self-pay medical practice. We do not bill health insurance. Patients are responsible for payment of services provided by our practice.
Our transparent pricing allows us to focus on individualized care without relying on insurance networks or third-party coverage decisions. We recommend maintaining health insurance for medical needs outside our practice, including hospital care, emergency services, and other specialty care.

Yes. HSA and FSA funds may be used for eligible medical expenses, subject to the rules of your individual plan.

NW Sports & Performance Medicine is a self-pay medical practice. We do not bill OHP (Oregon Health Plan), Medicare, or Medicare Advantage plans.
If you have OHP or Medicare coverage, please let us know before scheduling or receiving care. Additional requirements may apply when a patient with government-sponsored coverage chooses to receive services from a self-pay practice.
For OHP members, Oregon law requires specific disclosures and, in certain circumstances, a written Agreement to Pay before a patient may privately pay for a service.
For Medicare beneficiaries, physicians who do not bill Medicare must follow Medicare's applicable enrollment or opt-out requirements and, when required, use a private contract with the patient.
If you have OHP or Medicare and are considering care at our practice, please contact us before your appointment so we can provide the appropriate information and documentation.

Although we do not bill insurance directly, we can provide documentation of your visit and payment, including a superbill when appropriate, that you may submit to your insurance company for possible out-of-network reimbursement.
Any reimbursement is determined by your individual health plan. We cannot guarantee that your insurance company will reimburse you.

A Focused Sports Medicine Evaluation is designed for a specific concern or injury. A Comprehensive Sports Medicine Evaluation provides more time for complex, chronic, recurrent, or multifactorial concerns, multiple issues, performance goals, or a more extensive review of your medical history, imaging, or laboratory results.
Acute Injury & Illness visits are designed for straightforward illnesses and minor injuries that can be evaluated and treated in an outpatient setting.

our visit begins with a detailed discussion of your symptoms, medical history, activities, and goals, followed by an appropriate physical examination.
Depending on your condition, your evaluation may also include diagnostic musculoskeletal ultrasound or review of existing imaging and laboratory results. We will then discuss your diagnosis, treatment options, and a plan that fits your goals and circumstances.
We have the ability to order labs and imaging and to in-house limited labs/swabs and in-house procedures if indicated.

No. You do not need a referral to schedule a visit.
If you have relevant medical records, imaging, or laboratory results from another clinician, you are welcome to provide them before or during your visit.

Yes. We care for children and adolescents with sports, musculoskeletal, and activity-related concerns. Care is tailored to the patient's age, activity level, stage of development, and individual goals.
For acute illness and injury care, our clinic is not optimized for young infants less than 6 month, who may require specialized equipment or pediatric resources that are not available in our clinic. If you are unsure whether your child's needs are appropriate for our clinic, please contact us before scheduling.

No. NW Sports & Performance Medicine is an outpatient sports and performance medicine practice that also provides care for selected acute illnesses and minor injuries.
We are not a full-service urgent care center or emergency department, and we do not have the equipment or resources to manage every medical condition. The need for additional testing, monitoring, or a higher level of care cannot always be determined before an in-person evaluation.
If your evaluation indicates that your condition requires care beyond what our clinic can safely provide, we will recommend referral to an appropriate higher level of care.
For medical emergencies or potentially life-threatening symptoms, call 911 or go directly to the nearest emergency department.

Our goal is to provide appropriate outpatient care while recognizing when additional resources are needed.
If, during your evaluation, we determine that your condition requires testing, monitoring, treatment, or specialty services that are not available in our clinic, we will recommend an appropriate next step, which may include referral to urgent care, an emergency department, or another medical facility.

Yes. Follow-up visits are available when ongoing evaluation or treatment is appropriate.
We also offer structured programs for patients who want additional support with return to activity, fitness, performance, or other longer-term goals.

We ask for at least 24 hours' notice when canceling or rescheduling an appointment.
A $50 late cancellation fee may apply when less than 24 hours' notice is provided or an appointment is missed.
